Daily Nurse Visit Program

Assist patient with the administration of:

• Oral medication

• Insulin

• Inhalers

• Eye drops

• Injections (Vit. B12, Biweekly or Monthly Psychiatric Injections)

Monitor Patient’s:

• Blood sugar

• Blood pressure

• Weight

• Vital signs

*All medication administration times, readings and vitals are monitored through Electronic MAR for each patient*

 

Report to:

Pharmacist/Treating Physician on a Daily/Weekly basis to make regimen changes as directed.
